This survey allows an individual to report a bicycling accident in detail. The
information collected resides in a research database. After obtaining a large sample
of survey entries, a formal risk assessment study will
be conducted. The results will be made available for educational and advocacy purposes.

is recorded.
Your participation is completely voluntary.
This survey is open to cyclists from around the world of all ages, cycling
ability and experience.
It is ok to complete the survey on behalf of some else. When entering
a record for someone else, 'you' refers to the cyclist whom experienced
the accident.
Please fill out as much of the information as possible.
Please be honest. If you are responsible for the accident, please indicate
so. The point is to learn from our mistakes. By remaining honest, others
will benefit from such collective experience.
